Complete the REQUIRED forms (click to download and complete electronically).

• 2012 Mission Trip Application
• 2012 Mission Trip Application for special teams going to Fondwa with leaders from their churches
• 2012 Mission Trip Application for Duke Teams (only)
• 2012 Mission Trip Liability Waiver - This is required for all trip applicants and should be included with all submitted forms and documents

The Next Steps

1. Email your form to Justin Davis, FHM's mission team coordinator, if you have used the electronic signature.

   Otherwise, mail both completed forms along with your $200 deposit (deposit is for regular teams; it is waived for medical relief teams in March) to:

Pay your $200 deposit by :

• Mailing a check to the address above
• Calling FHM at (919) 382-5500 with your credit card information (type of card, number and expiration date).
• Submitting a credit or debit card payment on-line through PayPal. (You must mail us the forms.)
